Gather your required enrollment documents:<\/p>\n<p>a. Student proof of birth (birth certificate, passport, baptism certificate)<\/p>\n<p>b. Student Immunization records\u00a0(with first &amp; last names, date of birth, dates of all shots given, physician stamp for\u00a0each shot; if immunization has been transcribed, physician stamps for all shots).<\/p>\n<ul>\n<li><a href=\"https:\/\/fremontunified.org\/about\/instructional-services\/student-support-services\/enrollment-registration\/immunization-requirements\/\" target=\"_self\" rel=\"noopener\">Immunization Requirements<\/a><\/li>\n<\/ul>\n<p>c. Residency Verification &#8211; Two (2) of the following are required (documents need to be current, within the last 30- 45 days):<\/p>\n<ul>\n<li>\u00a0Lease agreement or mortgage statement<\/li>\n<li>\u00a0Utility bill<\/li>\n<li>\u00a0Pay stub<\/li>\n<li>\u00a0DMV documentation (car registration, renewal notice)<\/li>\n<li>\u00a0Official Government mailing<\/li>\n<\/ul>\n<p><strong>***Caregiver Affidavits\/Declarations of Residency:<\/strong>\u00a0If you are in need of a Caregiver Affidavit and\/or a Declaration of Residency, you must have these forms completed prior to enrollment in an FUSD elementary and secondary school.\u00a0<a href=\"https:\/\/fremontunified.org\/about\/instructional-services\/student-support-services\/enrollment-registration\/caregiver-affidavit-and-declaration-of-residency\/\" target=\"_blank\" rel=\"noopener\">To determine if you are in need a Caregiver Affidavit and\/or a Declaration of Residency<\/a>.<\/p>\n<p>It is important that you scan the verification\u00a0documents to your computer\/mobile device prior to\u00a0 beginning\u00a0the online enrollment application. You will need to access these documents while completing the\u00a0enrollment application. Please save each document as its own document as you will upload\u00a0them in different sections of the online application (residency documents can be canned\u00a0together). Please name the file with the last name and type; please do not use non- alpha\u00a0numeric characters such as commas, periods.<\/p>\n<p>Once you begin the process, you will receive an email from Infinite Campus with instructions on\u00a0completing the online enrollment.<\/p>\n<p>2.\u00a0Complete an\u00a0online\u00a0\u00a0Application\u00a0Link-\u00a0<a href=\"https:\/\/fremontunifiedca.infinitecampus.org\/campus\/OLRLogin\/fremont\" target=\"_blank\" rel=\"noopener\">Application<\/a>. If you have more than one student you wish to enroll, you will need to complete one enrollment application per student.\u00a0Please be sure to\u00a0select the appropriate school year in your online application.\u00a0An email is required to complete the online enrollment process. If you do not have an email address, please call at 510-657-2350 ext. 12295. After you submit the application, you will receive an email from Infinite Campus with instructions to schedule an appointment for a virtual meeting to finalize the enrollment.<\/p>\n<p>FUSD Enrollment Page: <a href=\"https:\/\/fremontunified.org\/enroll\">fremontunified.org\/enroll<\/a><\/p>\n<p>3. After completing the online form you will receive an email directing you to set up a virtual\u00a0enrollment\u00a0 appointment. This email should be sent immediately after you &#8220;submit&#8221;. Please\u00a0check your spam\/junk you &#8220;submit&#8221;. Please\u00a0check your spam\/junk folder if you do not see this email.<\/p>\n<p>4. Have your Virtual Appointment: During this appointment you will need to present enrollment is not complete until after this appointment. If you are unable to attend a virtual appointment and would prefer an\u00a0 in-person appointment, call at 510-657-2350 ext. 12295.<\/p>\n<p>5.\u00a0 Once enrollment is complete, you will be directed to the Special Education Department to pick up a Child Find\/Referral Packet.<\/p>\n<p>6.\u00a0 Submit the completed packet to Glankler Early Learning Center located at 39207 Sundale Drive.<\/p>\n<p>7.
