Attention Parents\/Guardians of 5th Grade Students:<\/span>

Warm Springs participates in girls & boys basketball. \u00a05th-grade students are able to try out for the teams. Due to the large amount of 5th grade classes we have, we are not able to have 3rd or 4th graders try out.\u00a0<\/span><\/p>

Girls Team Tryouts<\/span>\u00a0– Thursday, November 13, 2025 – 2:45pm to 3:45pm<\/span><\/p>

Boys Team Tryouts<\/span>\u00a0– Monday, November 17, 2025 – 2:45pm to 3:45pm<\/span><\/p>

If your 5th grade student is interested in trying out for the 2025 – 2026 Warm Springs Basketball Team, they will need to submit:<\/span><\/p>

1)\u00a0<\/span>Preparticipation Physical Evaluation<\/span><\/a>\u00a0form prior to trying out. A<\/span>\u00a0physician will need to complete this form<\/span>. Please plan ahead as medical appointments tend to take awhile to obtain.\u00a0<\/span><\/p>2)\u00a0Basketball Permission Slip<\/u><\/a> that requires both a parent and teacher’s signature.<\/span>

If the Preparticipation Physical Evaluation form and the Basketball Permission Slip form is not turned into the office by 3:30pm on November 12th, the player will NOT be allowed to tryout. There will be no exceptions.<\/span><\/u><\/strong>
