Student-Athletes must have all paperwork submitted <\/strong>before the first day of tryouts\/practice.<\/h4>\n<h4>**COACHES CANNOT COLLECT PAPERWORK**<\/h4>\n<strong>Below are the 2 required forms to participate in athletics (which you will also see in Home Campus as you move through the clearance process). Insurance info can be updated anytime. All portions must be completed including [insurance carrier and policy number] to receive clearance.<\/li>\n<li>Physicals are good for one calendar year from date on physical form.
