If our former student is now attending your school, please send your official request for records via fax or email.

If you are not a school and are requesting records (ie Attendance, Discipline,etc), please send an email to jdamewood@fusdk12.net. Make sure that you include a release from the parents allowing you to get the information. Without a release from parents, your request will not be fulfilled.
