On February 3rd, you will receive an email from InformedK12 regarding the required Annual Residency Verification. The email will contain a pre-filled form with your student's information.
You will need to upload two (2) current proofs of residency (dated within the last 30 days). Please note that your ID or driver’s license is not an acceptable proof of residency.
Acceptable documents include:
Mortgage statement
Rental property contract/lease
Utility service contract/statement
Pay stub
Voter registration
Correspondence from any government agency
Declaration of Residency form signed by the student's parent/guardian
After you sign and submit the form, it will be forwarded to the school site for processing.
