General – Forms Required To Participate

All students are Kennedy have the opportunity to participate in sports.  There are certain requirements that students must abide by in order to participate though.  Sports are a privilege, they are not a right.  

The following forms must be filled out by every student that would like to participate:

Fremont Unified School District requires that all students have insurance to participate in athletics.  The form below provides those whom do not have insurance to purchase insurance on a yearly basis.
