Medical Information

Emergency Cards

It is the parents responsibility to make sure the school has current emergency information. Families will be provided with an emergency card at Maze Day or upon enrollment. The office should be notified of changes in phone numbers, addresses and emergency contacts (including contacts other than the parent or guardian). Accurate information will enable us to make contact quickly in the case of an emergency.

Medication

Medical treatment is the responsibility of the parent and the family physician.  In most instances, the parent, with the help of the family physician, is urged to work out a schedule of giving medication outside of school hours.  If it does become necessary for a student to take any form of medication at school, a form signed by the physician and the parent must be presented to the school secretary.  Over-the-counter drugs such as aspirin or Tylenol may not be administered at school without proper form.

At the end of the school year, all medication left for use at school must be picked up by the last day.

Nursing Services

The school nurse is here one day per week.  The school nurse will conduct various screenings during the school year.

Medical Emergencies

When a student is injured, 911 will be called if the student is unconscious, bleeding profusely, not breathing, or has a serious injury where he/she cannot move without assistance.  Parents are notified, as well as the school nurse when she is at a different school.
