All required paperwork needed to participate in athletics at WHS is ONLY accepted through the online platform known as Home Campus. Student-Athletes must have all paperwork submitted before the first day of tryouts/practice.

**COACHES CANNOT COLLECT PAPERWORK**

Below are the 2 required forms to participate in athletics (which you will also see in Home Campus as you move through the clearance process). Follow the link for information and instructions on how to access Home Campus and complete the clearance process.

• Emergency Contact/Medical Insurance is complete through Home Campus and only needs to be completed once with the application. Insurance info can be updated anytime. All portions must be completed including [insurance carrier and policy number] to receive clearance.
• Physicals are good for one calendar year from date on physical form. Must be uploaded each school year, even if a physical was uploaded during the pervious school year.
• Athletic Academic Eligibility form needs to be signed off in Home Campus.
