How to File a Claim and Form (Liability)
If you become a part of an accident for which you believe the Fremont Unified School District has caused a loss to your person or property, you have the option of completing and filing a liability claim form with the district. Each claim filed will be reviewed by the district's Risk Management department and a determination will be made to accept or reject your claim based on an investigation of your accident. You will be notified via U.S. Mail of any decision regarding your claim.
Carefully read and complete the FUSD LIABILITY CLAIM FORM and return it to the Risk Management department at F.U.S.D. Please note that claim forms must be signed and submitted via U.S. Mail or hand delivered to the Risk Management department.
