Medication Policy

Medication Policy For FUSD (Ed Code 49423) - If a pupil is required to take medication during the school day, please fill out a Hold Harmless Form, available from the school office.  The form includes:

1.   A written statement from the physician detailing the method and time schedules for the medication

AND

2.   A written request from the parent that the medication be given at school.  In addition, the medication must be in the original prescription container with the label intact.  Please send only the amount to be administered at school with up to a maximum of 30 days quantity of medication.

The above policy is for the protection of all students and staff.  At no time is a student to carry medication in his/her lunch bag, pocket, backpack, etc., unless a form signed by parent and physician is filed in our office.  For information please contact Linda Pricer, Attendance Clerk, at 794-0392 ext. 51411.
